Athletic Department Forms

These forms include a sports information form, liability release and emergency contact information form.


Health Forms

All freshmen, transfers, juniors and student-athletes who did not participate at MSOE last year must obtain a physical examination and complete the MSOE Health Services records.

Also note that copies of the completed health forms must go to the Heath Services Office and to the Athletic Department office.
