Medical Forms
All MSOE NCAA athletes must complete medical and insurance forms annually.
All Freshman and transfers must complete the History form, obtain a physical examination, and complete the Insurance Information form prior to participation in MSOE Athletics.
All returning athletes must complete the Returning Athlete questionnaire and Insurance Information form to be eligible to participate.
