Medical Forms


All MSOE NCAA athletes must complete annual forms in order to participate in intercollegiate practices and contests.

New this year are the Sickle Cell form and Insurance Acknowledgement form for new and returning athletes.

Freshman / Transfer / First time athletes

  • All freshman, transfers, and students who have not previously participated in intercollegiate NCAA sports at MSOE must complete the MSOE New Athlete/Transfer Medical Form Packet.
  • All Physicals are to be on MSOE FORMS ONLY. Students are encouraged to received physicals from their own primary physician.
  • High School physicals (WIAA, IHSA, etc.) are not accepted.
  • All physicals must be completed within 3-months of the student-athlete’s first day of classes (after May 31).
  • A front and back copy of your insurance card must be attached to the other forms.

Returning MSOE Athletes

  • All who are returning MSOE student-athletes and have previously participated are to complete the Returning Athlete Medical Form Packet
  • ALL athletes must complete an Annual Insurance Information Form.

Insurance Information

The NCAA and MSOE require all MSOE student-athletes to provide evidence of primary insurance that covers injuries that occur during participation. No student-athlete will be allowed to participate in any way until evidence of current insurance coverage is on file with MSOE Sports Medicine.

  • Complete the insurance form in the respective Medical Form Packet.
  • Turn in a front and back copy of your current insurance card
  • If you do not have primary insurance contact Sports Medicine and we can help direct you.

Failure to complete all sections of each form will result in and incomplete form and will prohibit any athlete from engaging in contests and practices.
