This Online Registration Form is only for businesses, educational institutions, and other organizations not associated with the Milwaukee School of Engineering. Such organizations must fill out the form below and send it in, or they must register in-person at MSOE, in order to check out videos from the collection. Students, faculty, and staff of MSOE do not need to fill out this form.
As part of the registration process for a business, university, or other organization, a Key Contact from the organization must be designated. The Key Contact is the person who will serve as the primary contact for their organization for billing purposes. More than one Contact from an organization may be designated (but only ONE Key Contact). However, as a general rule, no more than three (3) contacts (including the Key Contact) may be designated. If more Contacts are desired, please contact the MSOE Library at (414) 277-7180.
The form is an email request for registering to borrow the videos. When submitted, the request is forwarded to the MSOE's Business Excellence Consortium (BEC).
After sending in the form, you then need to send or deliver a $250
borrowing fee to the BEC. The address

The $250 borrowing fee is an annual fee required each time that you renew your registration. If you send in your fee, be sure to adequately identify your organization so that the fee can be matched to the registration.
When both the registration and the borrowing fee are received by the BEC, you will receive an acknowledgment.
After this registration form is submitted to the BEC, and after the annual $250 borrowing fee is received by the BEC, the registration form additionally serves as a binding Corporate Lending Agreement between MSOE and the Contact person(s) listed below, together with the Contact person(s') organization.
When registration and fee have been received by the BEC, the BEC will notify the MSOE Library, which in turn will issue an MSOE Library card.
A library card (good for one year) will be issued to all Contacts. Contacts need to pick up this card in-person in the MSOE Library. The library card can only be used to check out videos from the MSOE MMAC Business and Management Video Collection. If a Contact cannot pick up the card in-person, the Contact should communicate with the MSOE Library (at 414-277-7180) in order to work out an alternative method for obtaining the card.
A valid library card must be issued before any checkouts of videos.
By completely and accurately filling in the online registration form, and by sending it in to MSOE, together with the $250 annual borrowing fee, the Contact and his or her organization agrees to the provisions, policies, and procedures detailed in the MSOE MMAC Business and Management Video list.
