Health Forms

All students new to the university, including transfer students are required to have a Health Record on file with Health Services before the start of their first term at MSOE. Answer the following questions to get your custom health record form in PDF format.

In order to view your health form, you must have Adobe Reader installed. If you do not already have it, get Adobe Reader.

You must answer all questions in order to receive the correct set of forms.

1. What year is this for you at MSOE?
    Transfer students: select ‘1’ to complete your general forms.
    Nursing students: please return to this page and select the year you are transferring in for additional form I-Physical Exam.  You do not need to complete additional forms F-TB Screen and H-Health Record Update as you have covered these in your general health record.
    Upperclassmen: if you have never completed a health record, please select '1'.
2. Are you a nursing student?
3. Are you a nursing faculty member?


ATHLETES: If you are planning to be, or currently are, a member of an MSOE NCAA Athletic team please visit the MSOE sports medicine page to fill out the required athletics forms.
    Please note that Club sports do not apply, i.e., Rugby, Tae Kwon Do, Waterski & Wakeboard, Weightlifting, Xtreme Club, Bowling, Cycling, Fencing, Judo, Paintball, and SPHEC clubs.
